Employee Comment Form

Employees may submit comments anonymously or may share their name and contact information for follow up purposes. To make an anonymous comment, do not complete the items labeled “optional”.

Comments will be directed to the Shared Governance Council and will be reviewed by the committee at their next scheduled meeting. Responses will be posted on the “Responses to Employee Comments” webpage.
