All Students Must Answer – Please choose if you want John A. Logan College to report your enrollment to VA. If you choose yes, you will be certified and VA will process your monthly stipend payment. If you choose no, you will not be certified and VA will NOT process your monthly payment. NOTE: If you have Chapter 33 Post 9/11 GI Bill® your tuition and fees will not be paid if you choose to not be certified. *

Please choose if you want John A. Logan College to charge you Illinois State Scholarship. If you choose yes, points will be deducted from your Illinois State Scholarship. If you choose no, points will not be deducted from your Illinois State Scholarship and you will be responsible for paying your tuition and fees.

If I have stipend payment problems, I should call the JALC Veterans Resource Office to ensure my certification was submitted correctly. The JALC Veterans Resource Office WILL NOT be able to tell me where my stipend payment is, how much it will be, or why I have not received it. I can call 1-888-442-4551 to speak to a live VA Department of Education Representative. If I am using Ch31 Vocational Rehabilitation, I need to call my Ch31 counselor directly; the STL Processing hotline will not be able to assist me.
REMINDER: Chapter 1606 and Chapter 30 users must verify their attendance on the last day of every month. I can do this by registering for and using my W.A.V.E. website or by calling 1-877-823-2378. My file number is my social security number.
CH33 Users ONLY: Effective December 2021 I must verify my attendance with Veterans Affairs the last day of every month that I am attending classes. I can do this by calling 1-888-442-4551, using W.A.V.E. website, or signing up for VA’s text verification service. My file number is my social security number.
If I am eligible for any Financial Aid or Scholarships (Pell Grant, MAP Grant, Mantracon, etc.)my Bursar’s Bill must have a balance of zero before I receive a refund.
If I have not turned in my eligibility letter for any benefit that covers my tuition and fees, my Financial Aid or Scholarship will be used until it is received. I will receive a refund once I turn in my eligibility letter.
There are 100% drop dates and withdraw dates that I need to be familiar with because they can affect the benefits I use. They can be found on the JALC Instructional calendar on the www.jalc.edu website.
I must complete this form or contact the Veterans Coordinator personally in order to be certified each semester.
