Appeal for Extraordinary Circumstances
The John A. Logan College Promise Scholarship Program offers the opportunity to appeal eligibility in the event of extraordinary circumstances. Extraordinary circumstances are defined as significant events that were unforeseen and completely out of the family’s control. Examples of extraordinary circumstances may include but are not limited to: death in the family, serious accident, sudden extended illness, or moving into the school district after freshman year.
Required Documentation:
This completed form.
Written statement explaining your appeal
The written statement from the student is your opportunity to explain your particular situation and detail why you need consideration. Include any relevant details that would help the committee understand the circumstances surrounding your appeal. Maximum 250-word response.
Verifiable supporting documentation.
For attendance, obtain a copy of your official attendance record from your high school attendance office. Any supporting documentation (i.e. doctors note, out of state travel due to funeral, etc.) must be attached to support your appeal. Please note, an “excused” absence from high school does not mean the same for Promise. You must explain and submit documentation to corroborate your absences. If you are unsure of what documentation to submit, please contact the Promise Scholarship Coordinator.
For GPA, required documentation includes a current copy of your transcript that shows GPA calculation (unofficial transcript is acceptable). Any supporting documentation (i.e. doctors note, out of state travel due to funeral, etc.) may also be attached to support your appeal.
For persistence, required documentation includes evidence that demonstrates your inability to progress to the next grade level, and your plan for remediating this. Work with your high school counselor and create and attach a plan describing how you will get back on track within the school year.
For moving into the district, required documentation includes your transcript from your previous high school showing a minimum 2.0 GPA, 90% attendance, and any applicable dual credit, early college, or transitional courses.
